Macronodular hyperplasia with marked adrenal enlargement (110 gm) with 5.5 cm dominant nodule, extension into periadrenal adipose tissue, myelolipomatous and lipomatous metaplasia, always bilateral, often delay (in years) of bilateral presentation. ACTH-independent.
