35M with PMH gynecomastia now with new, large aortocaval mass c/f malignancy;
Retroperitoneal mass FNA.
Positive for neoplastic cells- metastatic seminoma
Note: The biopsy shows sheets of discohesive, uniform appearing but degenerated tumor cells with prominent nucleoli. Immunohistochemical stains show the tumor cells to be positive for OCT3/4, b-HCG (weak, patchy) and negative for CD30 and AFP. This immunoprofile in the context of a testicular mass supports the diagnosis.
