54-year-old man presented with orange colored plaques on his neck which was biopsied. The patient has a history of left shin pain six years prior and subsequent imaging studies revealed an expansile lytic lesion in the distal tibia. The biopsy of the bone lesion showed fibrosis, necrosis, acute and chronic inflammation and focal xanthomatous inflammation.
