38 y.o. woman with a 2 month hx of positional vertigo, L facial weakness, tinnitus, and left anterior tongue numbness
Integrated diagnosis:

DIFFUSE MIDLINE GLIOMA, H3K27M-MUTANT

Histological diagnosis: Anaplastic astrocytoma

WHO (integrated histological and molecular) grade: IV

Molecular information: H3K27M: MUTANT (immunohistochemistry) IDH: wildtype (IDH1/2 PCR sequencing) P53: negative in tumor cells (immunohistochemistry, consistent with wildtype) ATRX: nuclear expression retained (immunohistochemistry, consistent with wildtype)

MGMT promoter: unmethylated (methylation specific PCR)
