Case #9 - The patient is a 46-year-old female with a past medical history significant for type II diabetes mellitus complicated by end-stage renal disease requiring peritoneal dialysis, peripheral vascular disease, and atrial fibrillation maintained on coumadin. She has had multiple recent falls. She presents with chief complaint of painful indurated plaques on bilateral thighs. Relevant laboratory results: Creatinine 12.32 mg/dL (reference interval 0.6-1.50 mg/dL), calcium 8.5 mg/dL (reference interval 8.5-10.5 mg/dL), phosphate 5.2 mg/dL (reference interval 2.6-4.5 mg/dL), PTH 538 pg/mL (reference interval 10-60 pg/mL). The lesion is pictured in clinical photograph #9 below. A punch biopsy is performed.
