80 yo man with patch/plaques then tumors diagnosed as MF, then radiated. Now ? recurrent disease on hand and back.
Skin, left hand: Primary cutaneous T-cell lymphoma, CD8+, with high proliferation index. IHC shows the atypical lymphocytes are CD3+ CD2+ CD8+ granzyme B+, TIA1+, with loss of CD5 and CD7 (subset), CD4-, CD30-. Ki67 is close to 100%. Positive TCR beta and TCR gamma gene rearrangements.
