64 year old man with blurry vision, diplopia, and a mass involving the right ethmoid sinus
Diagnosis
Adenoid cystic carcinoma Malignant salivary gland tumor with myoepithelial and ductal differentiation. Primarily affects adults Histological patterns: tubular, cribriform, solid Hyalinized/myxoid/mucoid filled spaces Perineural invasion is almost universal (pain, facial palsy...) Characteristic molecular: t(6;9) MYB-NFIB MYB IHC can be useful but is not 100% specific
