Clinical history

76 y/o F with history of polycythemia vera and an enlarged spleen containing 3 masses, measuring up to 8.5 cm.

Diffuse large B-cell lymphoma
  • 60s, M:F= 1-2:1
  • Solitary or multiple large (>5 cm) well-demarcated, white masses surrounded by normal spleen
  • Primary DLBCL of spleen is not common (<1% of all lymphomas)
  • Primary: good prognosis (80% five year survival); disseminated: poorer prognosis



Splenic marginal zone lymphoma

  • >50 y/o, M=F
  • <2% of lymphoid neoplasms
  • White lesions in a diffuse, miliary type pattern
  • Indolent clinical course, ~10% transform to large cell lymphoma




  • Most common benign neoplasm in spleen
  • All ages, usually adults
  • Solitary or multiple nodules
  • Red/purple, spongy mass with well-defined borders but unencapsulated
