Fibroma
  • Typically unilateral
  • Bilateral in Gorlin syndrome (basal cell nevus syndrome)
  • Usually > 40 years old
  • Average size: 6 cm in diameter
  • Well-circumscribed
  • Smooth, lobulated outer surface
  • Firm, tan-yellow to gray-white cut surface

Metastatic gastric carcinoma (Krukenberg)

Metastatic breast carcinoma

4 cm right-sided adnexal mass in a 49yo
  • Usually bilateral
  • Solid, multinodular, white cut surface
  • Wide size range, but usually <5 cm
  • Metastatic gastric carcinoma (Krukenberg tumor) can be especially challenging to distinguish from a fibroma grossly

Adnexal Leiomyoma

15 cm left-sided adnexal mass in a 45yo

Leiomyoma with hydropic change

  • Typically unilateral
  • Usually >40 years old
  • Well-circumscribed
  • Firm with a white-gray cut surface with a whorled appearance
  • Although commonly solid, may show areas of myxoid and/or (pseudo)cystic change

Infarcted Brenner tumor

15 cm right-sided adnexal mass in a 45yo
  • Usually unilateral
  • <10% bilateral
  • Usually 30-60 years old
  • Typically <2 cm (often an incidental finding)
  • ~10% are >10cm
  • Well-circumscribed
  • Solid, firm, white to pale yellow cut surface
  • Nodular, lobulated
